Self-Screening Health Questions for visiting TVCC locations (including Autism- Entry to School classrooms)

Please consider the following before you leave home for your appointment…

Have you or your child/youth:

  • Had a fever in the last 24 hours?
  • Had gastrointestinal issues (vomiting, diarrhea, nausea) in the last 48 hours?
  • Been exposed to measles in the last 21 days*?

If you answered YES to any of these questions, please DO NOT COME TO YOUR APPOINTMENT.  Please contact your clinician directly or call 519.685.8700 / 1.866.590.8822, and we will re-book your appointment or switch to a virtual appointment if possible.

*If you and your child are fully vaccinated against measles and the visit is deemed essential, your clinician will provide you further direction.

What you need to know before your appointment…

  • Do not come to your appointment if you or your child/youth has signs or symptoms of fever, vomiting, diarrhea, nausea.
  • Do not come to your appointment if you or your child/youth have been exposed to measles in the past 21 days and has not received two measles-containing vaccines.
  • Do not come to your appointment if you or your child/youth have symptoms of a cold, flu, covid 19, or other respiratory virus.
  • If you or your child/youth/client has been sick in the last 10 days please consider wearing a mask.  We can provide you with a mask if needed.
  • Your Clinician will minimize risk of spreading infections in their practice. Should you or your child/youth/client have symptoms of an acute infection, your appointment may be ended and will be re-scheduled.

What you can expect from us…

  • All our staff monitor their own health for cold, flu, covid and respiratory infection symptoms. Staff experiencing symptoms do not come to work in any of our locations or community sites.
  • Staff returning to work once symptoms are improving will wear a medical mask at all times until the end of day 10 from symptom onset.
  • Staff will wear personal protective equipment (PPE) as  appropriate for the client's visit.
  • Treatment spaces are cleaned after each use.
  • High-touch surfaces are cleaned frequently.
  • Our facilities are cleaned thoroughly every day.
