My child / youth has been discharged but I need more help

Start with your school or community clinician (if you have one). They might be able to help.

Send a new referral (use the Make a Referral button on the right). Please indicate that you have been seen by ACS in the past. Your child / youth will be placed on a shorter waitlist.

being leased? Contact your ACS clinician, or our program assistant at: 1-519-685-8700 ext. 58774

owned (after leasing)? Submit a re-referral, using the Make a Referral button on the right. Please indicate you have been seen by ACS in the past. You will be prioritized if your child / youth no longer has a functioning communication system.

What if my child / youth is 18 or older?

Please self-refer to the nearest adult AAC Clinic, Communication Technology Clinic (CTC) at Chicopee (Waterloo Regional Health Network) in Kitchener, ON. They can be contacted at ctc [at] wrhn.ca (ctc[at]wrhn[dot]ca) or 519-749-4300 ext. 7278

How do I book a consultation?

Consultations are for community clinicians (SLPs and OTs) who would like support to introduce or progress augmentative communication with a child/youth. They may request a consultation by selecting the 'consultation' on the ACS referral questionnaire.
