Who is eligible to apply?
Individuals who:
- Are between the ages of 15 & 30.
- Are no longer attending secondary school.
- Have a disability which significantly affects access to existing community programs and services
- Are motivated and willing to commit time to reach their personalized goals
- live in London and Middlesex County
