provides services to children and youth age birth to 19 who have physical, communication or developmental needs and reside in Southwestern Ontario. Our services are provided primarily in our clients’ homes, daycares, schools, and/or recreational and leisure facilities. We support and encourage children, youth and their families to be active participants in their communities. Our services are highly individualized and based on each child’s and families’ needs and preferences. Using a collaborative goal-setting model with children and youth, their families and daycare/school personnel/community service providers, a coordinated and integrated plan is developed. A variety of service delivery options are available to families including assessments, direct therapy, consultation, counseling, parent education and support groups, and community education. Services are based on a “Life Needs Model of Service” and through a family-centred approach:
