The following information will be used to process your order. Payment may be made by Cheque, or Credit Card (according to your preference). Following submission of this form, your order will be sent for processing immediately. You will receive an e-mail confirming your order with a unique order number. If you select "paying by cheque", you may print off the email you receive and mail it to TVCC with your Cheque (please quote your order # on your cheque). Your order will be shipped 4 - 6 weeks following receipt of your cheque.

If you select "paying by credit card", you may mail or fax a copy of the e-mail you receive with the credit card information completed, or phone in your credit card information (again quoting your unique order number).
